1. Initial Assessment: The process begins with thoroughly reviewing the veteran’s medical history and current health challenges.

2. Case Conference: A team of healthcare experts discusses the veteran’s specific needs.  Led by a GP, this team may include professionals from various fields, such as:

   1. Psychology

   2. Physiotherapy

   3. Exercise Physiology

   4. Podiatry

   5. Nutrition

3. Personalised Plan Development: The team creates a detailed plan addressing immediate and long-term health goals.

4. Connection to Services:  The experienced team at Medibase works with each veteran to implement the plan and link them to the right allied health providers to effectively work towards improving their health outcomes.

5. Ongoing Monitoring: Regular check-ins and updates to the plan ensure it continues to meet the veteran’s evolving needs.

Who Is Eligible?

The program is available to all veterans who hold a White Card with Non-Liability Health Care (NLHC) activated as the minimum eligibility criterion.
